This is the new national incident response system and will replace the current serious incident reporting system. It consists of a number of helpful tools, designed to help people understand what has gone wrong when an incident has happened and a quite different way of understanding this. This approach takes an appreciative approach, has an expectation that the person/ family impacted are fully involved, along will other stakeholders and takes a system approach to safety. This new system has been tested by a number of organisations prior to publication and there is an expectation currently that there will be an expectation that this new framework will be implemented in organisations by Sept 2023.

This new framework looks to have far more potential to allow transparent understanding and learning when care goes wrong.
